The hardest game to ever grace the NES is Namco's version of Star Wars.
Namco's Star Wars is simply impossible without an emulator. 3 lives, no continues, no cheats to choose a level to start at. And the levels are hard as hell.
I don't care how hard a game is, as long as I can restart on the same level again and again, I can clear it. But this...
If you can clear it without Save States, you are the greatest gamer in history.
Oh, and good video. :)
Edit: I have cleared Battletoads and Ghosts and Goblins without cheats nor savestates. But there is no way in hell I'm ever trying to clear Namco's Star Wars on my famicom ever again. It's just that impossible. That's a shame too, because it's pretty good (though weird).

Does the syrup you are "drinking" have any ammonium cloride in it?
I am asking because I've been told that in some countries it is used in cough syrups.
(In Finland we make candies out of it.)

Thanks LordKat, I got this game through a trade with a ''friend'' when I was like 8 years old, and I never got past the second stage.
Now, I can see that I didn't miss anything...

Oh and ... have you ever played wiz´s & war´s III ? I loved that game even more . althought this is one of the games i never played through as a child . ( i even did turtles 1 and mega man 2 and 3 , without speaking a single word of english , so its not my "skill" responsible for that )
always died in the upper knight regions or got lost in the magical caverns after loosing track of where the heck i am , as soon as you dropped down 20 screens and glowing undefeatable monsterheads block your way and you are kind of "imprisoned" :(
